To make the therapy experience as simple and easy as possible, we sign, store, and transmit all of our documents electronically via a HIPAA compliant, secure client platform. Although we do not typically print/fax/scan physical forms and documents, we have provided hard copies on this page for you to review and download. These forms include our office policies, the HIPAA Privacy Policy Notice, Limits of Confidentiality, and other documents that are also accessible for download via the client portal.
